Ink and Moles in the Field: Shield, Skirt or Send Away


Your treatment plan? A Q-switched pulse doesn't care. It goes where you point it. Put a tattoo or a mole in that field and the pigment grabs the energy first. Hot spot. You can avoid it three ways: shield it, skirt it, or send the client home. Consider this your field guide for all three.

1. When ink or a dark mole sits inside your treatment field

Any dark pigment in the beam path turns into a competing chromophore. Melanin in a mole. Carbon in tattoo ink. Iron oxide in cosmetic tattoo. They soak up Q-switched energy faster than surrounding skin. Then heat. Blisters. Sometimes a scar. Sometimes permanent pigment change. You don't want any of that.

Picture a tiny lightning rod on the skin. The pulse hunts for the darkest point and concentrates there. That's the physics. Your job? Keep the rod out of the beam.

2. The three options: shield, skirt, or send away

Exactly three safe moves exist when ink or a mole sits in or near the field. No fourth option. Don't cover it. Don't lower the fluence and hope. Pick one.

Shield

Shielding means you cover that lesion with an opaque, light-blocking material. It can't burn or melt under the pulse. Use a white or metallic shield made for laser work. Wet gauze? Not enough. Paper tape? Not enough. The shield has to be a few millimeters larger than the lesion. Flush with the skin. Keep it dry. If it lifts or shifts, the pigment gets exposed.

A round metal or ceramic laser shield handles flat moles and small tattoos well. For a raised mole, a cut-out shield with a central window may be safer. It's harder to keep flush, though. Raised lesions? Better to skirt them or send them away.

Skirt

Skirting means you treat around the lesion and leave a margin of untouched skin. How much? At least 2 to 3 mm from the visible edge of ink or mole. Dense black ink needs more. Irregular borders need more. The margin accounts for scatter. Q-switched light scatters under the skin. Even if the beam spot never touches the pigment, scattered photons can still heat it. A 3 mm margin usually covers it. Darker skin or very dense ink? Use 5 mm.

You don't treat the lesion itself when you skirt. That's fine. You're not doing tattoo removal here. You're doing another treatment in the same area. The lesion is a bystander. Leave it alone.

Send away

Send away means you don't treat that area at all that day. Say the mole or tattoo sits dead center in the field. Shielding or skirting would leave a visibly untreated patch, right? The better call is to reschedule or treat a different area. If the lesion looks suspicious, send the client to a dermatologist before any laser work. More on that below.

3. Why covering is never an option

You might think a layer of white tape or thick zinc oxide will do the job. It won't. Cover the lesion with something translucent or partially reflective and you create two problems. First, the cover itself heats up. That causes a thermal burn on top of the mole. Second, some covers scatter light unpredictably. That scattered light can still reach the pigment, just at an angle. You end up with partial, uneven heating. That's worse than a clean skip. Patchy darkening can happen. So can an inflammatory response without full clearance. So can a scar.

Back in 1993, Anderson and colleagues reported cosmetic tattoo ink darkening after Q-switched and pulsed laser treatment. Ross and colleagues linked some cases to titanium dioxide in the ink. Fire over a covered tattoo and you're gambling with that darkening reaction. Don't.

4. Suspicious moles: send away every time

See any of the following on a mole? Don't shield it, don't skirt it, don't treat over it. Refer the client to a dermatologist for evaluation before your next session. Use the ABCDE rule as a quick screen.

  • Asymmetry: one half doesn't match the other.
  • Border irregularity: edges look ragged, notched, or blurred.
  • Color variegation: more than one shade of brown, black, or red.
  • Diameter greater than 6 mm, roughly the size of a pencil eraser.
  • Evolving: the mole has changed in size, shape, or color.

Refer too if the mole bleeds, itches, or crusts on its own. A family history of melanoma raises the bar. When in doubt, send away. The laser can wait. A missed melanoma cannot.

5. Photo annotation workflow: mark the field before you fire

You need a photo record showing exactly where the lesion is and what you did. Do this before the client gets on the table. Use a consistent system.

  1. Take a high-resolution photo of the treatment area under consistent lighting.
  2. Mark the lesion on that photo with a circle or arrow. Use annotation software, not a pen on the skin.
  3. Note the lesion type: tattoo, cosmetic tattoo, flat mole, raised mole, suspicious mole.
  4. Write down your chosen action: shield, skirt, or send away.
  5. If shielding, photograph the shield in place before the first pulse.
  6. If skirting, mark the intended margin on the skin with a surgical marker and photograph it.
  7. Save the annotated photo to the client record with the date and treatment area.

6. If the pulse hits ink by accident

Accidents happen. A client moves. A shield slips. You fire before you check. Here's the sequence.

Stop immediately

Don't fire another pulse over that area. Remove the handpiece from the skin. Call it to the team.

Cool the site

Apply cool saline compresses or a cold pack wrapped in a cloth. Don't rub. Don't pop any blister that forms. Cooling limits the thermal injury.

Assess and document

Photograph the area immediately. Write down the fluence, spot size, and pulse count from that session. Note any history of keloids or pigment disorders. Document the incident in the chart. Show the client what happened. Be direct.

Follow up

Schedule a wound check in 48 to 72 hours. An intact blister? Leave it alone. If it breaks, apply a thin layer of petrolatum and a nonstick dressing. See signs of infection? Refer out. If the pigment darkens over the following weeks, refer to a dermatologist. Don't attempt to correct it with more laser that day.

Bencini and colleagues noted that tattoo removal outcomes depend on many variables. Accidental partial treatment is one variable you don't want to add. A clean miss always beats a messy hit.

7. Parameter reasoning when shielding or skirting

Don't raise the fluence to compensate for an untreated spot when you shield or skirt. The rest of the field still gets the planned dose. That untreated spot will heal or fade at its own pace. Acceptable. Raise the fluence and you increase the risk of burning surrounding skin. Not acceptable.

If you need to work close to the lesion edge, use a smaller spot size. A smaller spot gives you more control over the beam edge. It also reduces the scattering halo. Always do a test patch on a nearby area before treating a full field that includes a shielded lesion. The test patch confirms your settings. It also gives you a baseline for the skin's response. See our test patching protocol for the step by step.

8. Documentation and consent

Your consent form should mention that tattoos and moles inside the treatment field will be shielded, skirted, or referred. It should state that you don't treat over such lesions without dermatology clearance. It should ask the client to point out any new or changing moles before the session starts. That one question saves you a lot of trouble.

After the session, write one line in the chart. "Two flat moles in left cheek field shielded with metal discs. No laser over lesions." That line becomes your best defense. It also forces you to notice the lesions every time. That's the whole point.

Ink and moles aren't obstacles to work around. They're warning signs to respect. Shield them, skirt them, or send the client away. Never cover them. The pulse will find the darkness. Make sure you find it first.

FAQ

Frequently asked questions

Can I laser over a tattoo if I use a cooling gel?

No. Cooling gel doesn't block the light. It may actually spread the beam. You still get energy to the ink. Avoid it.

How much margin should I leave around a mole when skirting?

At least 2 to 3 mm for flat, stable moles. Use 5 mm for dense ink or dark skin. More if the border looks irregular. When in doubt, refer.

What do I do if a client refuses to let me shield a mole and insists I treat over it?

You decline. Document the refusal and the reason. Don't treat over an unshielded mole without dermatology clearance. Your license matters more than one session.

Can I remove a normal mole with a Q-switched laser?

No. A Q-switched laser isn't a mole removal tool. Even benign moles should be evaluated before any laser treatment. If removal is indicated, refer for appropriate excision or dermoscopy-guided treatment.
